You will support Team members with their caseload of clients with complex Co-existing Mental Health and Substance Use disorders, who have compounding issues and/or other needs; including:

complex co-existing mental health and substance use conditions addressing their substance misuse issues using assertive in-reach, multi-agency partnership working & treatment plansa dedicated pathway of care
non-compliance or engagement with support services neurodevelopmental conditionslearning disabilitieschallenging personality disorderspoor physical healthlack of community &/or family support, unsuitable living conditionschallenges from multiple services involvement safeguarding or exploitation concerns
